Terms & Conditions and Intake Form

The MITS Residential Terms and Conditions form is required before any work starts, especially when we are working on hard drives and other related devices that contain data.  Please complete the form, which guides you on where to sign.
The MITS Computer Intake Form is used for gathering initial issues regarding the repair, information needed to complete a new customer entry and additional information.

New clients, please complete the form below for the MITS Residential Terms and Conditions. Please read over the document and if you agree, sign the document, along with your printed name. If you are representing a business, please fill out the business name on the Client line, and your position in the Title line.

For all clients, new and returning, please fill out as much of the MITS Computer Intake Form as you can.  This form gathers your name, address, phone number and email, along with information on your computer, the issues you are experiencing and any other information you would like to pass to us.  We will also add estimates if we have them.
